When creating custom fields within a customised contact form, how do I find
out what the Visual Basic (either VBA or VBScrpt) equivalent is for the
custom fields when including them in a piece of code.

Can anyone help?

All custom fields can be accessed by name through the ContactItem.UserProperties collection. See http://www.outlookcode.com/article.aspx?ID=38
